I was injured May 28, 1993 while working for Pizza Hut. I had just come back from a delivery, noticed trash on ground by the back door; so while bending over to pick it up, another employee rushing out to make a delivery, slammed the door into my head. Since that date I have had tremendous headaches, neck pain (which I was getting Upper Epidural injections, pain medications, had therapy, and trigger point injections to relieve some of the pain.
Recently, due to fact that my adjuster thru Kemper Ins. has for the past few years been usually 9 times out of 10 unavailable, will not return phone calls, takes her sweet time authorizing prescribed medication, and that's if she knows what the medicine is for, if not, she will not authorize it) and if it happens to be a long holiday weekend your out of luck until next business day, hopefully. All this is documented by faxes (they the ins. carrier forced me to get a fax to protect myself and document evidence of when and what date etc.)
Until then it was just her word against mine. even had a Mediation hearing where as at the time I was getting at home injections to eliminate so many er runs (which since 8-17-99 the girl who gave the injections has not been paid one penny even though my physician authorized the injections at home) at the Mediation we brought up the fact that the medication for injections at home came in a vial (like diabetics insulin) so that was why the need for syringes (dr.prescribed) and Kemper refused to authorize.
The adjuster, said, "oh I did not know, lie, had faxed several times explaining it to her) showed where I had to pay for syringes myself (how else was I to get medicine into my body) she stated Dec 4, 2001 (in front of mediator, Kempers lawyer, and my own lawyer, that Kemper would authorize and pay for syringes from then on, all documented) still they have not paid any reimbursement for what I laid out for syringes, nor authorized them. Now its March 2002, my treating physician has refused to treat me anymore due to fact (documented) of non-cooperation.
